Primary Care Reimagined

Direct Care is a healthcare model in which patients pay a per service fee or monthly membership fee directly to the medical practice for access to certain healthcare services. This model allows providers to focus on personalized, relationship-based care without the administrative limitations of insurance billing. 

Patients may still use their health insurance for labs, imaging and prescription medications!

The monthly membership fee includes access to the following services when medically appropriate:

Direct Primary Care Services

• Comprehensive primary care visits
• Preventive care and wellness visits
• Acute illness visits
• Chronic disease management

Communication & Access

• Secure messaging  anytime with the provider via phone,  text message, email, patient portal
• Telephone consultations
• Telehealth visits when appropriate
• Same or next-day appointments exclusively to DPC members

Care Coordination

• Assistance coordinating referrals to specialists
• Guidance on laboratory testing and imaging
• Prescription management

Wellness & Integrative Care

• Lifestyle and nutrition counseling
• Preventive health planning
• Review of supplements and wellness strategies
  

 Services Not Included in Membership
The membership fee does not include certain services, including but not limited to:

• Laboratory testing
• EKG
• Imaging (X-rays, CT scans, MRI, etc.)
• Prescription medications
• Specialist consultations
• Hospital services
• Procedures requiring outside facilities
• IV vitamin therapy
• Hormone therapy medications
• Peptide therapy
• Weight loss medications
• Supplements or nutraceuticals
• Specialty functional medicine testing

Direct Care Monthly Membership Fee: includes all scheduled office visits, urgent care visits and integrative primary care services, including hormone and metabolic optimization. It does not include medical weight management, but it can be added for an additional fee. 

Same day or next business day appointments guaranteed via in person or telehealth. 24/7 access to your provider for URGENT matters. 
Our Direct Care Subscription program is limited to a small number of patients due to 24/7 provider access and the ability to offer same/next day appointments that is exclusively afforded to Direct care subscription members.
